ABSTRACT
PURPOSE: To assess tilt, decentration, and axial stability of the RayOne EMV toric intraocular lens (TIOL) (Rayner Intraocular Lenses Ltd) over a 6-month follow-up period.
METHODS: This was a single-center, prospective, interventional clinical trial at the Department of Ophthalmology and Optometry, Medical University of Vienna, Vienna, Austria. A total of 119 eyes from 79 patients with age-related cataract were implanted with a RayOne EMV TIOL. Tilt, decentration, and aqueous depth (AQD) were measured using anterior segment optical coherence tomography (Casia 2; Tomey Corporation) preoperatively, at 1 hour, 1 week, and 6 months postoperatively. Individual changes in tilt and decentration were evaluated using vector analysis.
RESULTS: Mean absolute tilt and decentration values were 5.30 ± 1.38° and 0.14 ± 0.10 mm for the crystalline lens, 5.28 ± 1.27° and 0.21 ± 0.11 mm for the TIOL at 1 hour, 5.22 ± 1.25° and 0.20 ± 0.10 mm at 1 week, 5.33° ± 1.81° (range: 0.90 to 11.10°) and 0.22 ± 0.11 mm (range: 0.04 to 0.57 mm) at 6 months. Mean changes in tilt and decentration from preoperatively to 6 months were 1.59 ± 1.66° (range: 0.09 to 8.43°) and 0.17 ± 0.09 mm (range: 0.02 to 0.47 mm). Seven TIOLs showed a tilt change over 5° within 6 months. A posterior axial shift of 0.13 ± 0.14 mm (range: -0.06 to 0.69 mm) was observed between 1 week and 6 months and correlated with anterior fibrosis severity (P < .001).
CONCLUSIONS: Although mean absolute tilt and decentration values were low, in some cases, a pronounced tilt and posterior axial hyperopic shift occurred. Excessive anterior fibrosis development was responsible for these changes.
